This Privacy Policy applies to all personal information collected by Future Natural Health via the website located at Futurenaturalhealth.com.

What is 'personal information?

(a) The Privacy Act 1988 (Cth) currently defines 'personal information as meaning information or an opinion about an identified individual or an individual who is reasonably identifiable:

(ii) whether the information or opinion is recorded in a material form or not.

What information do we collect?

(a) The kind of personal information that we collect from you will depend on how you use the website. The personal information which we collect and hold about you may include name, date of birth, gender, addresses, contact details, medical information including medical history, medications, allergies, adverse events, social and lifestyle history, diet history, family history and risk factors.

How we collect and store your personal information

(a) Our practice may collect your personal information in several different ways:

(i) When you make your first appointment personal information is collected via your registration form.

(ii) During the course of providing medical services, we may collect further personal information.

(iii) We may collect personal information when you visit our website, send us an email or SMS, telephone us, make an online appointment or communicate with us using social media.

(iv) In some circumstances personal information may also be collected from other sources if it is not practical or reasonable to collect it from you directly. This may include information from: your guardian or responsible person

(b) We also collect cookies from your computer which enable us to tell when you use the website and also to help customise your website experience. As a general rule, however, it is not possible to identify you personally from our use of cookies.

GDPR

In some circumstances, the GDPR provides additional protection to individuals located in Europe. The fact that you may be located in Europe does not, however, on its own entitle you to protection under the GDPR. Our website does not specifically target customers located in the European Union and we do not monitor the behaviour of individuals in the European Union, and accordingly, the GDPR does not apply.

Sharing of personal information

(a) We may disclose your personal data to our insurers and/or professional advisers insofar as reasonably necessary for the purposes of obtaining or maintaining insurance coverage, managing risks, obtaining professional advice, or the establishment, exercise or defence of legal claims, whether in court proceedings or in an administrative or out-of-court procedure.

(b) We may disclose your personal data where such disclosure is necessary for compliance with a legal obligation to which we are subject, or in order to protect your vital interests or the vital interests of another natural person.

By using our website, you consent to the receipt of direct marketing material. Our direct marketing material will include a simple means by which you can request not to receive further communication of this nature.

Access and correction

Australian Privacy Principle 12 permits you to obtain access to the personal information we hold about you in certain circumstances, and Australian Privacy Principle 13 allows you to correct inaccurate personal information subject to certain exceptions. If you have a complaint concerning the manner in which we maintain the privacy of your personal information, please contact us as set out below. All complaints will be considered by Future Natural Health and we may seek further information from you to clarify your concerns. If we agree that your complaint is well-founded, we will, in consultation with you, take appropriate steps to rectify the problem. If you remain dissatisfied with the outcome, you may refer the matter to the Office of the Australian Information Commissioner.

Overseas transfer

Your personal information will not be disclosed to recipients outside Australia unless you expressly request us to do so. If you request us to transfer your personal information to an overseas recipient, the overseas recipient will not be required to comply with the Australian Privacy Principles and we will not be liable for any mishandling of your information in such circumstances.
